Why organizations engage IFS4hh
Navigate change, quantify impact, and make better-informed decisions.
Since 2008, John Reisinger and IFS4hh have helped home health and hospice organizations interpret complexity—and understand how those interpretations shape decisions.
The underlying challenge
Organizational outcomes are produced by connected decisions.
Home health and hospice organizations operate in an environment of constant change. Reimbursement methodologies evolve, regulatory requirements expand, operational challenges intensify, and leadership teams are routinely asked to make consequential decisions with incomplete information.
IFS4hh was developed to address a fundamental problem underlying this environment: how organizations interpret complexity—and how those interpretations shape decisions.
Clinical decisions
Operational processes
Reimbursement systems
Financial outcomes
A unified analytical system
Understand the relationships—not only the individual results.
The IFS4hh Analytical Methodology integrates domains that are often treated separately—clinical decision-making, care planning, resource utilization, operational performance, reimbursement systems, and financial outcomes—into a single structured framework.
Rather than analyzing these areas in isolation, the methodology focuses on the relationships between them. This allows organizations to better understand how changes in one area propagate across the organization and ultimately affect outcomes.
Decision
A clinical, financial, operational, or reimbursement choice is made.
Interaction
The decision influences other functions and organizational systems.
Impact
Resource use, performance, reimbursement, and outcomes begin to change.
Insight
Structured analysis makes those relationships more visible before and after implementation.
Experience built through industry change
A methodology forged across reimbursement environments.
I began my career as a Medicare Auditor in 1992, moved to provider-side leadership with a multi-state home health/hospice organization in 1997, and have advised home health and hospice organizations as a consultant since 2008. That experience has provided a firsthand perspective on the major evolutions in home health reimbursement—from Cost-Based Reimbursement through IPS and PPS, including leading provider-side transitions to both systems, and later helping clients prepare for the transition to PDGM in 2020.
Medicare audit and compliance
A detailed foundation in reimbursement, regulation, Cost Reporting, and provider operations.
Provider-side leadership
Operational experience during one of the industry’s most transformative periods.
Anticipating impact
Budgeting, forecasting, modeling, and reimbursement analysis designed to evaluate change in advance.
An evolving system
Structured analytical thinking applied to current clinical, operational, regulatory, and financial complexity.
Beyond traditional financial consulting
Focus on what produces the outcomes.
IFS4hh is not defined by traditional accounting, reporting, or advisory services. Its focus is on understanding the relationships that produce organizational outcomes—not simply measuring those outcomes after the fact.
That analytical approach has been applied across a wide range of organizational needs.
The analytical methodology
Four integrated dimensions.
Each dimension supports the same objective: improving decision-making by improving understanding of how organizational processes produce outcomes.
Experience
Grounded in decades of exposure to Medicare systems, provider operations, consulting environments, and industry change.
Analysis
Focused on interpreting what organizational data reveals about clinical, financial, operational, and reimbursement activities.
Methodology
A structured and repeatable approach for evaluating decisions and quantifying their organizational impact.
Insight
The translation of analytical findings into actionable understanding that supports better-informed decisions.
Better understanding leads to better decisions.
Better decisions lead to better outcomes.
A practical decision framework
Move from observation to organizational meaning.
The methodology organizes complex situations around three direct questions.
What is happening?
Understand current organizational conditions and identify the issues that require attention.
Why is it happening?
Identify the relationships among clinical decisions, operational processes, reimbursement structures, resource utilization, and financial outcomes.
What does it mean for our organization?
Translate those relationships into actionable organizational insight.
What is happening → Why is it happening → What does it mean for our organization?
Decision-support methodologies
Turn analytical thinking into practical application.
Over time, the IFS4hh Analytical Methodology has been operationalized into structured decision-support systems, analytical models, and planning tools designed for the home health and hospice environment.
Each is designed to help organizations understand the consequences of decisions before those decisions are reflected in operational, clinical, or financial outcomes.

A coaching philosophy
Build internal decision capability—not dependency.
Engagements emphasize shared understanding, applied reasoning, and structured thinking so that leadership teams, clinicians, and operational staff can apply similar logic independently in the future.
The objective is a measurable shift in organizational capability—from reliance on external interpretation to internal application of structured analytical thinking.
The objective is not simply to resolve today’s problem, but to strengthen the organization’s ability to address future challenges.
Bringing it together
Engage at the level most relevant to the current need.
IFS4hh maintains a consistent underlying analytical methodology across different forms of support.
