Why the Home Health Spending Adjustment May Not Reflect the Impact on Your Agency
Each year, when CMS releases the Home Health PPS Proposed and Final Rules, one of the most widely discussed figures is the projected change in Home Health Spending (HHS).
Industry publications reference it. Consultants discuss it. Associations analyze it. Agency leaders often look to it for insight into what the rule may mean for their organizations.
Unfortunately, one of the most common misconceptions in home health reimbursement is the belief that the projected change in Home Health Spending is also a reliable estimate of how Medicare reimbursement will change for an individual agency.
In reality, those are two very different things.
What Is Home Health Spending?
Home Health Spending is CMS’s estimate of how a reimbursement change may affect Medicare spending across the home health industry as a whole.
It is intended to reflect the aggregate impact of reimbursement changes across all Medicare-certified home health agencies.
As an industry-wide measure, Home Health Spending can provide useful insight into CMS’s expectations regarding the overall financial impact of a Proposed or Final Rule.
However, it was never intended to serve as a prediction of how reimbursement will change for any specific agency.
The Problem With Industry-Wide Estimates
When a Proposed or Final Rule is released, it is common to hear statements such as:
“CMS is increasing payments by 2%.”
or
“The rule is expected to reduce Home Health Spending by 3%.”
While those statements may accurately describe CMS’s estimate of industry-wide spending, they do not necessarily describe what an individual agency will experience.
The challenge is that many agency leaders assume the industry-wide estimate applies equally to every organization.
It does not.
An agency may experience results that are significantly different from the projected change in Home Health Spending, either positively or negatively.
As a result, relying solely on industry-wide projections can create a false sense of certainty regarding future financial performance.
The Question Agency Leaders Should Be Asking
Rather than focusing exclusively on the projected change in Home Health Spending, agency leadership should be asking a different question:
- “What is the expected impact on my agency?”
That question shifts the discussion from industry averages to agency-specific financial performance.
After all, budgets, staffing decisions, strategic planning initiatives, and operational decisions are based on an agency’s actual reimbursement, not industry-wide estimates.
Understanding the potential impact on your own organization is often far more valuable than understanding the projected impact on the industry as a whole.
Why This Matters
Even relatively modest reimbursement changes can have a meaningful impact on agency profitability.
Many organizations continue to face increasing labor costs, rising operating expenses, competition for clinical staff, and ongoing regulatory pressures.
Under these circumstances, assumptions based solely on industry-wide projections can lead to inaccurate budgeting, unrealistic expectations, and missed opportunities for proactive planning.
The earlier agency leadership understands the potential impact of reimbursement changes, the more time they have to evaluate operational, financial, and strategic options.
Looking Beyond the Headlines
The Home Health PPS rulemaking process will continue to evolve as CMS updates reimbursement methodologies and payment rates.
While the projected change in Home Health Spending will undoubtedly continue to receive significant attention, agency leaders should remember that industry-wide estimates do not necessarily predict agency-specific results.
Successful agencies tend to be those that look beyond the headlines and focus on understanding how reimbursement changes may affect their own operations, financial performance, and long-term strategic objectives.
